A Deadline Extension Form and Claim Form must be received by the Administrator by the Ultimate Claims Deadline (January 4, 2024) under the Settlement Agreement. Please note that it is the Claimants' responsibility to ensure their claim is submitted before the Ultimate Claims Deadline.
